class='avia_textblock'  itemprop=\"text\" ><p>Angesichts der demografischen und epidemiologischen Entwicklungen steht die Gesundheitsversorgung in Deutschland vor gro\u00dfen Herausforderungen. Der Bev\u00f6lkerungsanteil \u00e4lterer und alleinlebender Menschen nimmt ebenso zu wie die Zahl chronischer Erkrankungen, gleichzeitig kehren Infektionskrankheiten zur\u00fcck oder erreichen ganz neue Dimensionen. Dabei gibt es einen engen Zusammenhang zwischen niedrigem sozialem Status und schlechter Gesundheit. Pr\u00e4vention und Gesundheitsf\u00f6rderung sind in Deutschland unterentwickelt.<br \/>\n<br class=\"avia-permanent-lb\" \/>Aber auch Ver\u00e4nderungen der Versorgungsstrukturen m\u00fcssen bew\u00e4ltigt werden: Zum einen gibt es ein enormes Verteilungsproblem bei niedergelassenen \u00c4rztinnen und \u00c4rzten. Zum anderen haben wir im internationalen Vergleich eine enorm hohe Anzahl von Krankenhausbetten und nicht zuletzt wegen \u00f6konomischer Fehlanreize eine au\u00dfergew\u00f6hnlich hohe Anzahl von Operationen (zum Beispiel in der Orthop\u00e4die). Der Gesetzgeber hat es bisher nicht geschafft, diesen Problemen wirksam zu begegnen. Das deutsche Gesundheitswesen ist zudem gekennzeichnet durch eine hohe Fragmentierung, die einen gro\u00dfen Teil der Versicherten \u00fcberfordert. Verst\u00e4rkt wird das Problem durch eine gering ausgepr\u00e4gte Gesundheitskompetenz (Health Literacy) (Hurrelmann 2020). Angesichts der nebeneinander bestehenden Unter\u2011, \u00dcber- und Fehlversorgung (SVR 2014) gibt es in Deutschland seit L\u00e4ngerem Debatten zur St\u00e4rkung der Prima\u0308rversorgung. Sie stellt im ambulanten Geschehen die zentrale Eintrittspforte ins Gesundheitswesen dar. Laut Sachversta\u0308ndigenrat (SVR 2014) ist ein niedrigschwelliger Zugang zu den Leistungen des Gesundheitswesens entscheidend f\u00fcr die Inanspruchnahme und Bedarfsgerechtigkeit eines Systems. Das ist besonders f\u00fcr vulnerable Gruppen (dazu geh\u00f6ren auch \u00e4ltere Menschen) und f\u00fcr Bev\u00f6lkerungsgruppen in strukturschwachen Regionen bedeutsam. Die Prima\u0308rversorgung muss auf dem Land, aber auch in st\u00e4dtisch gepr\u00e4gten benachteiligten Gebieten effizient und bedarfsgerecht gesichert werden. Gute Erreichbarkeit, integrierte Versorgungsangebote und ein breites Versorgungsspektrum bei hoher Qualit\u00e4t sind zentrale Ziele. Dadurch wird beispielsweise auch ein Verbleib im h\u00e4uslichen Umfeld bei beginnendem Pflege- und Unterstu\u0308tzungsbedarf erm\u00f6glicht. Die L\u00f6sung all dieser Herausforderungen w\u00fcrde erheblich zu mehr Gesundheit, Lebensqualit\u00e4t und Zufriedenheit beitragen. Die Community Health Nurses (CHN) arbeiten in kommunalen Gesundheitszentren, im Quartiersmanagement, bei ambulanten Pflegediensten, in Arztpraxen oder im \u00d6ffentlichen Gesundheitsdienst. CHN bieten einen niederschwelligen Zugang zu Gesundheitsf\u00f6rderung, Pr\u00e4vention und Beratung gerade f\u00fcr Menschen, die sonst schwer zu erreichen sind oder schwer Zugang zu guter Gesundheitsversorgung finden. Es besteht insbesondere eine Schnittmenge mit den Auswirkungen sozialer Faktoren auf die Gesundheit.<\/li>\n<li><strong>\u00d6ffentlicher Gesundheitsdienst:<\/strong> In einem in seinen Aufgaben reformierten \u00d6ffentlichen Gesundheitsdienst k\u00f6nnten CHN zu einer Perspektivenerweiterung beitragen. Entscheidend ist hierbei der niederschwellige Zugang zu den Leistungen der Versorgungssysteme. Denn vor allem in den strukturschwachen Gebieten wird dies zunehmend eine Herausforderung werden.<br \/>\n<br class=\"avia-permanent-lb\" \/>Das bisherige Verh\u00e4ltnis zwischen Medizin und Pflege muss sich ver\u00e4ndern. Es braucht eine neue Justierung von Zust\u00e4ndigkeiten und Kompetenzen. Bisher wird diese Diskussion eher berufsst\u00e4ndisch gef\u00fchrt. Versorgungsbed\u00fcrfnisse und vorhandene Kompetenzen spielen eine nachgeordnete Rolle. Dabei muss der Orientierungspunkt f\u00fcr die Gestaltung der Versorgung und der Zust\u00e4ndigkeiten allein der Versorgungsbedarf der Versicherten beziehungsweise die Kompetenz sowie Leistungsf\u00e4higkeit der Akteure sein. Mehrere WHO-Programme haben darauf hingewirkt, das Prinzip \u201eGesundheit f\u00fcr alle\u201c zu verwirklichen. Das hat in verschiedensten Auspr\u00e4gungen politischen Einfluss gehabt, aber an den Grundstrukturen der Versorgung in Deutschland nicht viel ver\u00e4ndert. Die Einf\u00fchrung von lokalen Gesundheitszentren unter Leitung der Kommunen bietet eine gro\u00dfe Chance, die Gesundheitsversorgung zu verbessern. \u00c4rzte k\u00f6nnten von Aufgaben entlastet werden und insbesondere die CHN bringen Kompetenzen und Perspektiven ein, die bisher vernachl\u00e4ssigte Bedarfslagen identifizieren und kompensieren helfen. Das Leistungsspektrum der Prim\u00e4rversorgung w\u00fcrde erweitert werden. Dies hat insbesondere im Zusammenhang mit chronischen Erkrankungen und Pflegebed\u00fcrftigkeit einen hohen Stellenwert. Hier kann u.a. das Selbstmanagement durch Beratung, Information, Anleitung und Patientenschulung gest\u00e4rkt werden.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die Etablierung von Community Health Nurses ist ein Prozess, der sicher einige Zeit in Anspruch nehmen wird. Gerade der wachsende Kompetenzzuwachs der Kommunen im Gesundheitsbereich, welche hier im Rahmen ihrer Aufgaben zur Sicherung der Daseinsvorsorge zust\u00e4ndig sind, bietet eine Chance, die bisher unbekannte Rolle der CHN einzuf\u00fchren. Aber auch der \u00f6ffentliche Gesundheitsdienst (\u00d6GD), der nach der Pandemie sicherlich eine Reform seiner Aufgaben und Ressourcen erfahren wird, bietet hier einen Ansatz zur Verbesserung der Versorgung.
