In jeder Branche und auch im Gesundheitswesen wird es immer h\u00e4ufiger verwendet, wenn auch die konkrete Bedeutung oft schwammig bleibt. Die in den letzten Jahren prominenter gewordene Diskussion um die Klimakrise fokussiert auf eine Definition von Nachhaltigkeit, die speziell die \u00f6kologische Modernisierung von Gesellschaften und Wirtschaftssystemen in den Mittelpunkt stellt. Doch die Weltkommission f\u00fcr Umwelt und Entwicklung hat bereits 1987 eine breitere Definition vorgelegt: \u201eEine nachhaltige Entwicklung entspricht den Bed\u00fcrfnissen der heutigen Generation, ohne die M\u00f6glichkeiten k\u00fcnftiger Generationen zu gef\u00e4hrden, ihre eigenen Bed\u00fcrfnisse zu befriedigen\u201c (Hauff 1987, S. 46). Diese breitere Definition zielt darauf ab, drei Bed\u00fcrfnisdimensionen miteinander zu vereinen: \u00f6konomisch, \u00f6kologisch und sozial. Diese gelten auch f\u00fcr das Gesundheitswesen.<\/p>\n<\/div><\/section><br \/>\n<section  class='av_textblock_section av-lbannn3t-9516c4247ddb9327c79bb4855017a977 '   itemscope=\"itemscope\" itemtype=\"https:\/\/schema.org\/CreativeWork\" ><div class='avia_textblock'  itemprop=\"text\" ><ul>\n<li><strong>\u00d6konomische Bed\u00fcrfnisse<\/strong> zielen auf die Bezahlbarkeit und die Effektivit\u00e4t der Gesundheitsversorgung ab, das hei\u00dft, wie viel Gesundheit je eingesetztem Euro \u201eproduziert\u201c wird (Graalmann et al. 2021a).<\/li>\n<li><strong>\u00d6kologische Bed\u00fcrfnisse<\/strong> beziehen sich auf die Umweltvertr\u00e4glichkeit, zum Beispiel mit Blick auf Ressourcenverbrauch und die Produktion von Emissionen und anderer Abf\u00e4lle sowie die Anpassungsf\u00e4higkeit etwa bei Hitze oder neuartigen Gesundheitsgefahren.<\/li>\n<li><strong>Soziale Bed\u00fcrfnisse<\/strong> bezeichnen vor allem einen gleichwertigen Zugang zur Gesundheitsversorgung unabh\u00e4ngig von Alter, Einkommen, Gesundheitszustand oder Wohnort der Patient:innen (vgl. Es geht am Ende um die Frage, ob die politischen, medizinischen und finanziellen Entscheidungen, die jetzt \u00fcber den Ressourceneinsatz getroffen werden, auch aus Sicht der folgenden Generation(en) \u2013 in 25 oder 50 Jahren \u2013 nachhaltig sind. Es spielt keine Rolle, ob die Babyboomer den Millennials geschadet haben oder die Generation Y nachhaltiger lebt als die Generation Z.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die internationale Gemeinschaft erzielte bei der nachhaltigen Ausrichtung unserer Gesellschaft bereits gro\u00dfe Fortschritte. Internationale Klimaabkommen, die Definition von Grenzwerten, die Determinierung von Nachhaltigkeitszielen f\u00fcr \u00f6konomische, \u00f6kologische und soziale Bed\u00fcrfnisse bieten eine gemeinschaftliche Grundlage, um einen nachhaltigen Ressourcenverbrauch zu erlangen. Dennoch gelingt es Akteuren in vielen L\u00e4ndern, Branchen und Institutionen nicht, ihre Entscheidungen stringent an diesen Zielen auszurichten \u2013 das trifft auch f\u00fcr das deutsche Gesundheitswesen zu. Aus \u00f6kologischer Sicht verbraucht das Gesundheitswesen heute 80 Prozent mehr Rohstoffe als Mitte der 1990er-Jahre und eine weitere Zunahme gilt als wahrscheinlich (Fraunhofer ISI 2021). Der Fachkr\u00e4ftemangel im Gesundheitswesen wird vor allem f\u00fcr die Pflege, aber auch f\u00fcr die \u00c4rzteschaft bereits seit Jahren klar prognostiziert, aktuell beziffert das Deutsche Krankenhausinstitut die L\u00fccke allein bei den Pflegepersonen auf 187.000 Vollzeitbesch\u00e4ftigte bis 2030. Und auch mit den finanziellen Ressourcen des Systems wird nicht nachhaltig umgegangen: Die strukturelle Finanzl\u00fccke in der Gesetzlichen Krankenversicherung (GKV) und der Sozialen Pflegeversicherung (SPV) w\u00e4chst trotz zahlreicher gesetzgeberischer Ma\u00dfnahmen best\u00e4ndig an. So steht die GKV aktuell f\u00fcr 2023 vor einem Finanzierungsdefizit von 17 Milliarden Euro, einem neuen H\u00f6chstwert. So stieg die Lebenserwartung in den letzten 60 Jahren international zwar von 54 auf 73 Jahre. Der Anteil der in Krankheit verbrachten Jahre stagniert aber bei 50 Prozent (s. Abb. 1, McKinsey Health Institute 2022). Die mit jeder Generation angestiegene Zahl der in Krankheit verbrachten Jahre f\u00fchrt im zweiten Schritt zu einem immer h\u00f6heren Ressourcenbedarf und -einsatz in allen Wirtschaftlich getriebene, kurzfristige Einsparungen gef\u00e4hrden den Zugang zu und die Qualit\u00e4t der Versorgung. Die kurzsichtige Jahreslogik erkl\u00e4rt zusammen mit Mengenanreizen auch die im internationalen Vergleich \u00fcberdurchschnittlich hohen Arztbesuche und operativen Eingriffe in Deutschland, die einen erh\u00f6hten Ressourcenverbrauch nach sich ziehen. Seien es Mengenvorgaben f\u00fcr Operationen an die \u00c4rzteschaft in den Krankenh\u00e4usern oder Ver\u00e4nderungsraten f\u00fcr die Verg\u00fctung der niedergelassenen \u00c4rzte. Verhandelt werden haupts\u00e4chlich prozentuale Steigerungsraten, die vergleichen, wie viel mehr Interventionen im Vergleich zum Vorjahr stattfinden sollen bzw. d\u00fcrfen. Dabei spielt es wirtschaftlich eine nachgelagerte Rolle, ob einzelne Interventionen medizinisch sinnvoll oder nachhaltig sind.<br \/>\n<br class=\"avia-permanent-lb\" \/>Ein anschauliches Beispiel ist die Investitionskostenspirale der Krankenh\u00e4user (Graalmann et al. 2021b). Diese erhalten seit Jahren zu geringe Investitionskosten von den zust\u00e4ndigen L\u00e4ndern. Um dennoch ein ausgeglichenes Finanzergebnis zu erreichen, versuchen die Krankenh\u00e4user die fehlenden Investitionsmittel \u00fcber zus\u00e4tzliche Eingriffe auszugleichen. Sie gehen \u201ein die Menge\u201c. Dadurch steigen nicht nur die Betriebskosten und der Ressourcenverbrauch, sondern auch der Investitionsbedarf. Die Investitionskostenspirale schraubt sich immer weiter nach oben. Inflationsbereinigt hat sich die F\u00f6rdersumme der L\u00e4nder seit 1991 halbiert (vgl. DKG 2022a). Krankenh\u00e4user, welche die notwendigen Mengen nicht generieren, laufen Gefahr, in die roten Zahlen zu rutschen. So machen nach Angaben der Deutschen Krankenhausgesellschaft aktuell etwa 60 Prozent der Krankenh\u00e4user in Deutschland Verluste (DKG Ein zun\u00e4chst teureres, aber umweltvertr\u00e4glicheres Material erh\u00f6ht die Betriebsausgaben ohne Erl\u00f6ssteigerung. Das Gleiche gilt f\u00fcr Investitionen, die in der kameralistischen Jahreslogik haupts\u00e4chlich als Aufw\u00e4nde wahrgenommen werden und weniger als Schaffung von langfristig wirtschaftlichen Verm\u00f6genswerten. So bedeutet eine Investition in eine nachhaltigere Energieversorgung eines Krankenhauses zun\u00e4chst Mehrkosten, obwohl langfristig die Betriebskosten durch geringere Stromkosten oder weniger Energiebedarf sinken. Stattdessen wirken Anforderungen, etwa im Bereich der Hygiene, mit dem Ziel einer besonders hohen Qualit\u00e4t bei gleichzeitiger Wirtschaftlichkeit dem Konzept \u00f6kologischer Nachhaltigkeit entgegen \u2013 man denke nur an den best\u00e4ndig steigenden Anteil von Einwegprodukten.<br \/>\n<br class=\"avia-permanent-lb\" \/>Dabei ist der Zusammenhang von steigendem Behandlungsbedarf und steigendem \u00f6kologischen Ressourcenverbrauch nicht zwingend. So zeigen Vergleiche mit dem National Health Service (NHS) in Gro\u00dfbritannien, dass es dort deutlich besser gelungen ist, Ressourcen nachhaltiger einzusetzen und den Ressourcenverbrauch deutlich zu senken (s. Kap. II.6). So konnte der CO2-Fu\u00dfabdruck des NHS seit 2010 um 30 Prozent reduziert werden. 2021 erweiterte der NHS seine Net-Zero-Strategie auch um die Lieferketten der Produzenten, die nun bis 2040 ebenfalls klimaneutral agieren m\u00fcssen, um den NHS noch beliefern zu k\u00f6nnen. Der NHS macht Nachhaltigkeit zur Voraussetzung f\u00fcr die Marktteilnahme. Das hat auch Wirkung auf die globalen M\u00e4rkte. F\u00fcr die Transformation des NHS werden vom Staat die notwendigen Investitionsmittel bereitgestellt. Davon kann in Deutschland bisher keine Rede sein. Hier treffen massive Bedarfe an Investitionen f\u00fcr mehr \u00f6kologische Nachhaltigkeit auf die beschriebene unzureichend finanzierte Infrastruktur. Ein Umdenken scheint auch nicht in Sicht. Denkbar w\u00e4re zum Beispiel die Nachjustierung des \u00a7 30 SGB IV, welcher Krankenkassen und ihre Aufsichtsbeh\u00f6rden heute auf eine stringente Auslegung der bestehenden Rechtsvorschriften beschr\u00e4nkt. Das f\u00fchrt bei der Entscheidung \u00fcber nachhaltige Alternativen regelm\u00e4\u00dfig zu Konflikten, wenn diese nicht den heute kurzfristigen Wirtschaftlichkeitszielen entsprechen. Denn beide Diskussionen greifen ineinander: Eine effektive, bedarfsgerechte und patientenorientierte Krankenhausstruktur, die \u00dcber- und Fehlversorgung ebenso abbaut wie die Unterversorgung an anderen Stellen, f\u00fchrt zu einem nachhaltigen Umgang mit Ressourcen auf allen Dimensionen und schafft einen Mehrwert an Gesundheit. Doch wie soll dieser umfassende Umbau finanziert werden?<br \/>\n<br class=\"avia-permanent-lb\" \/>In der aktuellen finanziellen Situation der GKV und unter bestehenden Anreizen ist eine solche Investition seitens der Akteure nicht zu erwarten. Um eine tats\u00e4chliche Transformation der Versorgungsstruktur zu erm\u00f6glichen, braucht daher eine Vorfinanzierung der Investitionen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Angesichts der Dringlichkeit des nachhaltigen Strukturwandels sollten die Mittel vielmehr \u00fcber \u00f6ffentliche Anleihen bereitgestellt werden. Der Zweck \u00f6ffentlicher Anleihen besteht in der Finanzierung von \u00f6ffentlichen Investitionen und Infrastruktur, also zur Deckung eines langfristigen Kapitalbedarfs, der nicht durch laufende Einnahmen gedeckt werden kann oder sollte. \u00d6ffentliche Anleihen k\u00f6nnen von Bund, L\u00e4ndern, Kommunen und Gebietsk\u00f6rperschaften f\u00fcr einen konkreten Investitionsbedarf ausgegeben werden. Da diese Investitionen der EU-Taxonomie f\u00fcr Environmental Social Governance (ESG) entsprechen, ist von einer entsprechenden Nachfrage am Kapitalmarkt auszugehen.<br \/>\n<br class=\"avia-permanent-lb\" \/>So k\u00f6nnten \u2013 analog zu den erfolgreichen gr\u00fcnen Anleihen \u2013 \u201ewei\u00dfe Anleihen\u201c von Bund und L\u00e4ndern ausgegeben werden, welche den nachhaltigen Umbau der Versorgungsinfrastruktur erm\u00f6glichen. Hierf\u00fcr muss Nachhaltigkeit im Gesundheitssystem systemisch verankert werden.
