Gemeint sind damit Investitionen in digitale Anwendungen f\u00fcr \u00c4rzte und Pflegepersonal, um effizientere, papierlose Prozesse oder einfachere Dokumentation zu erm\u00f6glichen. Das Problem: Die Ver\u00e4nderungsbereitschaft blieb insgesamt eher gering, da diejenigen, die die Anwendungen einsetzen sollten, nicht immer selbst einen Nutzen davon hatten. Ein verbesserter Datenaustausch beispielsweise hilft zwar, Doppeluntersuchungen zu vermeiden. Doch davon profitiert prim\u00e4r das Gesamtsystem, weniger der einzelne Arzt. Viele IT-Programme, egal ob das National Programme for IT im National Health Service (NHS) Gro\u00dfbritanniens oder die gematik in Deutschland, haben letztlich nicht den erhofften Nutzen gebracht.<br \/>\n<br class=\"avia-permanent-lb\" \/>Im Gegensatz zu eHealth ist im Bereich Digital Health \u2013 digitale Anwendungen f\u00fcr Patienten \u2013 lange Zeit recht wenig passiert. In Deutschland kommunizieren laut PraxisBarometer Digitalisierung der Kassen\u00e4rztlichen Bundesvereinigung immer noch 69% der Arztpraxen mehrheitlich \u00fcber Papier mit ihren Patienten (Albrecht et al. 2020). Services wie die Rezepterneuerung oder Online-Terminvereinbarung waren nur in 15% der Praxen verf\u00fcgbar. Und nur etwas mehr als jede zehnte Praxis war in der Lage, Patienten auf elektronischem Weg an Behandlungs- oder Vorsorgetermine zu erinnern oder Medikationspl\u00e4ne in digitaler Form bereitzustellen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Wie gro\u00df die Digitalisierungsl\u00fccke im Gesundheitswesen insgesamt ist, zeigt ein Vergleich mit anderen Branchen, wie dem Einzelhandel oder der Tourismusindustrie. In diesen beiden durch hohen Wettbewerb gekennzeichneten Branchen liegt der Digitalisierungsquotient weltweit gesehen bei 42 bzw. 39, im Gesundheitswesen hingegen lediglich bei 28 (s. Abb. 1). Der Quotient misst den Stand der Digitalisierung von Unternehmen, Organisationen und Branchen anhand der Kriterien Strategie, Organisation sowie F\u00e4higkeiten und Ziel ist es, alle Anforderungen der Kunden ganzheitlich abzudecken: Kunden k\u00f6nnen unterschiedliche Produkte oder Services kaufen oder nutzen, ohne dass sie daf\u00fcr das System verlassen m\u00fcssen. Typische Beispiele sind der Marktplatz von Amazon oder Buchungsplattformen wie booking.com, die eine nahtlose Kundenreise erm\u00f6glichen. Praktisch alle Befragten sch\u00e4tzten das Thema als sehr wichtig ein. 90% sind davon \u00fcberzeugt, dass Patienten eine integrierte, nahtlose Kundenreise gegen\u00fcber Einzell\u00f6sungen vorziehen w\u00fcrden. 86% gehen davon aus, dass \u00d6kosysteme einen hohen wirtschaftlichen Nutzen haben. Allerdings wird der Aufbau von \u00d6kosystemen nach Meinung der Befragten einige Zeit dauern. Immer mehr Patienten m\u00f6chten digitale Anwendungen nutzen \u2013 beispielsweise f\u00fcr die Vereinbarung eines Arzttermins oder einen Symptomcheck. Laut einer Bitkom-Umfrage im Mai 2021 fordern 71% der Bundesb\u00fcrger mehr Tempo beim Ausbau digitaler Angebote in der Medizin (2020: 65%) (Bitkom 2021). Gerade im Zuge der COVID-19-Pandemie ist das Vertrauen in digitale L\u00f6sungen deutlich gestiegen. So kamen laut dem McKinsey eHealth Monitor im ersten Quartal 2020 Gesundheits-Apps und -services auf fast zwei Mio. Downloads (McKinsey 2021). Das ist eine Verdopplung gegen\u00fcber dem Vorjahr. Nach kurzzeitigem R\u00fcckgang stieg die Anzahl der Downloads im zweiten Quartal 2021 sogar auf 2,4 Mio. an.<br \/>\n<br class=\"avia-permanent-lb\" \/>Der Nachfrage steht ein gro\u00dfes <strong>Angebot<\/strong> an digitalen Gesundheitsanwendungen gegen\u00fcber \u2013 allein im Google Play Store gibt es heute \u00fcber 50.000 entsprechende Apps, die sich mit einem Klick kostenlos oder f\u00fcr wenig Geld herunterladen lassen. In verschiedenen L\u00e4ndern wurden regulatorische \u00c4nderungen beschlossen, um solche Services \u201everschreibbar\u201c oder f\u00fcr die Leistungserbringer deutlich attraktiver zu machen. Dazu geh\u00f6rt beispielsweise, die Verg\u00fctung auf dem Niveau von Offline-Behandlungen zu halten oder Restriktionen hinsichtlich der Anzahl der abrechenbaren Leistungen abzubauen. Hier hat Deutschland eine wirkliche Vorreiterrolle mit der Einf\u00fchrung der DiGAs \u00fcbernommen. Nach einer Zulassung durch das Bundesinstitut f\u00fcr Arzneimittel und Medizinprodukte (BfArM) k\u00f6nnen diese Apps von \u00c4rzten verschrieben und in der Folge von Krankenkassen erstattet werden. Seit der Einf\u00fchrung des DiGA-Verzeichnisses wurden bis Mai 2021 insgesamt 72 digitale Gesundheitsanwendungen im BfArM beantragt.<br \/>\n<br class=\"avia-permanent-lb\" \/>Nicht zuletzt haben zahlreiche Untersuchungen den <strong>Nutzen<\/strong> der Digitalisierung f\u00fcr das Gesundheitssystem belegt. Allein in Deutschland betr\u00e4gt das Digitalisierungspotenzial rund 10% der gesamten Gesundheitsausgaben. Das entspricht ca. 34 Mrd. \u20ac. Gut zwei Drittel davon entfallen auf Anwendungen rund um den Arzt, ein Drittel auf Anwendungen rund um den Patienten (s. Abb. 2). Ein funktionierendes \u00d6kosystem begleitet Patienten durchgehend auf ihrem Weg durch das gesamte Gesundheitssystem \u2013 vom Checken der ersten Symptome \u00fcber die digitale und\/oder pers\u00f6nliche Behandlung durch den Arzt bis hin zur Abrechnung mit der Krankenkasse (s. Um eine durchgehende Patientenreise abbilden zu k\u00f6nnen, braucht es eine Plattform, mit der sich einzelne L\u00f6sungen verkn\u00fcpfen lassen. Dass eine solche Plattform hierzulande bis heute noch nicht existiert, liegt zum einen daran, dass in der Vergangenheit vor allem in eHealth, also in L\u00f6sungen f\u00fcr \u00c4rzte und Pflegepersonal, und nicht in L\u00f6sungen f\u00fcr Patienten investiert wurde. Zum anderen ist die Orchestrierung von digitalen Anwendungen allein kein tragf\u00e4higes Gesch\u00e4ftsmodell. Der Aufbau von solchen Plattformen macht nur f\u00fcr Akteure Sinn, die bereits \u00fcber ein tragf\u00e4higes und skalierbares Gesch\u00e4ftsmodell verf\u00fcgen, das durch das \u00d6kosystem weiter gest\u00e4rkt wird. Ihre Motivation: die Kosten zu reduzieren und Kunden zu gewinnen, die tendenziell einen gesunden Lifestyle pflegen. Der in S\u00fcdafrika ans\u00e4ssige Versicherer Discovery etwa hat schon vor mehr als 20 Jahren mit dem Aufbau eines Gesundheits\u00f6kosystems begonnen. Mit seinem digitalen Vitality-Programm motiviert er Kunden zu einem ges\u00fcnderen Leben \u2013 indem er Gesundheits- und Wellnessdaten auswertet, den Kunden Zugang zu einem Netzwerk relevanter Dienstleister bietet und finanzielle Anreize setzt. Auf diese Weise konnten die Kosten \u2013 auch unter Nutzung von Selektionseffekten \u2013 bis zu 14% reduziert werden. Ein anderes Beispiel ist das \u00d6kosystem Pulse von Prudential, welches in mehreren L\u00e4ndern Asiens digitale Gesundheitsservices anbietet. 70% der Nutzer sind noch keine Prudential-Kunden, k\u00f6nnen es aber per Mausklick werden. Fast zwei Mio. Vertr\u00e4ge sind so allein im ersten Jahr zustande gekommen \u2013 mit gesundheitsbewussten, digital orientierten und damit attraktiven Kunden. Ein letztes, noch neueres Beispiel: In der Schweiz ist im August 2021 die Gesundheitsplattform WELL an den Start gegangen. Die Beteiligten sind die Versicherer CSS und Visana, der Telemedizinanbieter Medi24 sowie der Arzneimittel- und Technologieanbieter Zur Rose. Sie k\u00f6nnen zusammen zwei Mio. Nutzer \u2013 ein Viertel der Schweizer Bev\u00f6lkerung \u2013 \u00fcber ihre bisherigen Kundenbeziehungen erreichen.<\/p>\n<p><br class=\"avia-permanent-lb\" \/>Auch f\u00fcr Leistungserbringer kann es sich lohnen, eigene \u00d6kosysteme aufzubauen, um mehr Patienten zu gewinnen. So hat die Krankenhauskette Apollo Hospitals in Indien die digitale Plattform Apollo 24|7 mit Services insbesondere rund um die Telemedizin mit dem Ziel aufgebaut, eine gr\u00f6\u00dfere Anzahl von Patienten zu versorgen. Die Online-Apotheke Zur Rose hat den gr\u00f6\u00dften Telemedizinanbieter in Deutschland \u2013 die TeleClinic \u2013 gekauft, um Patienten einen gr\u00f6\u00dferen Mehrwert als nur die reine Online-Apotheke zu bieten, n\u00e4mlich die ambitionierte Vision \u201eto create a world where everyone can manage their health with one click\u201c und ein Gesundheits\u00f6kosystem in mehreren L\u00e4ndern Europas aufzubauen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Und nat\u00fcrlich k\u00f6nnten auch Technologieanbieter wie Google oder Amazon ein Interesse am Aufbau digitaler Gesundheitssysteme haben. Zwar fehlt die N\u00e4he zum traditionellen Gesundheitswesen und viele Patienten d\u00fcrften den gro\u00dfen Tech-Spielern gerade beim Thema Gesundheit nicht unbedingt ihr Vertrauen schenken. Doch die Technologieanbieter sind auf der Suche nach neuen Wertquellen. So hat Amazon z.B. im Jahr 2018 die amerikanische Versandapotheke PillPack gekauft, um ihren Kunden die Lieferung verschreibungspflichtiger Medikamente nach Hause zu erm\u00f6glichen. Apple bietet mit HealthKit einen zentralen Ort zur Datenspeicherung von Gesundheits- und Fitnessdaten an, auf den HealthKit-kompatible Applikationen nach Zustimmung durch den Nutzer direkten Zugriff bekommen, um eine personalisierte Kundenreise anbieten zu k\u00f6nnen. Von den Vertretern des Gesundheitswesens, die McKinsey 2021 befragt hat, nennen 62% die gro\u00dfen Tech-Anbieter als wichtigste Treiber die Entwicklung. Aber auch Leistungserbringer (50%) und Start-ups (47%) werden in einer f\u00fchrenden Rolle gesehen (McKinsey 2021). Interessanterweise hat keiner der Akteure sich selbst als treibende Kraft genannt \u2013 es zeigt sich, dass viele Spieler die eigene m\u00f6gliche Rolle eher zu skeptisch sehen.<br \/>\n<br class=\"avia-permanent-lb\" \/>M\u00f6gliche Kandidaten brauchen in jedem Fall eine klare strategische Ausrichtung, verbunden mit der F\u00e4higkeit, \u00fcber den eigenen Tellerrand hinauszuschauen und mit anderen Akteuren auf Augenh\u00f6he zusammenzuarbeiten. Am Ende sollte die Beteiligung an einem \u00d6kosystem f\u00fcr alle Akteure im Gesundheitswesen eine Win-win-Situation sein (s. Es fehlt vor allem die Verbindung verschiedener bestehender Elemente. Ein Akteur m\u00fcsste die Initiative ergreifen, eine technische Infrastruktur bereitstellen (ID-Management f\u00fcr verschiedene digitale L\u00f6sungen, Consent Management) und Daten\u2011\/Analytics-Services aufbauen, die von allen Beteiligten genutzt werden k\u00f6nnen. Grundgedanke ist hier vor allem, dass Daten nahtlos \u00fcbernommen werden: Beantwortet ein Patient z.B. beim digitalen Symptomcheck schon alle relevanten Fragen und wird dann zur Telemedizin weitergeleitet, sollte sichergestellt sein, dass er sich nicht erneut einloggen und alle Fragen noch einmal beantworten muss. Die Arztkonsultation d\u00fcrfte in diesem \u00d6kosystem weiterhin zentraler Dreh- und Angelpunkt bleiben. Eine minimale Patientenreise k\u00f6nnte zun\u00e4chst den Weg \u00fcber die Telekonsultation bis zur Einl\u00f6sung eines Rezepts in einer Online-Apotheke abdecken.<br \/>\n<br class=\"avia-permanent-lb\" \/>Langfristig sind dann verschiedene Ausbaustufen unter Nutzung weiterer Netzwerkeffekte denkbar. So k\u00f6nnten beispielsweise Daten aus digitalen Aktivit\u00e4ten mit Daten aus der Regelversorgung verkn\u00fcpft werden. Versicherern w\u00e4re es so m\u00f6glich, Gelder verst\u00e4rkt dorthin zu lenken, wo der Nutzen am gr\u00f6\u00dften ist. Zeigt sich beispielsweise, dass sich durch Anwendung einer Diabetes-App die Zahl der Krankenhauseinweisungen reduziert wird, k\u00f6nnte im Sinne einer ergebnisabh\u00e4ngigen Verg\u00fctung ein Teil des so eingesparten Geldes an die App-Entwickler gehen. Solch ein Modell w\u00e4re fair und w\u00fcrde einen messbaren Mehrwert durch Innovation belohnen. Die klinische Forschung wiederum k\u00f6nnte durch Datenverkn\u00fcpfung besser verstehen, wie sich Alltagsaktivit\u00e4ten (z.B. Bewegung, Ern\u00e4hrung) auf die Gesundheit oder bestimmte Konditionen auswirken. Auch lie\u00dfen sich neue Ans\u00e4tze wie personalisierte Therapien zielgenauer verfolgen.
