Die aktuellen Zahlen zeigen: W\u00e4hrend die Digitalisierung in Praxen und Apotheken relativ z\u00fcgig voranschreitet, tun sich insbesondere Krankenh\u00e4user mit der Umstellung ihrer Systeme nach wie vor schwer. Gesundheitsdaten spielt der Ausbau der Telematikinfrastruktur (TI) weiterhin eine Schl\u00fcsselrolle. Bei den niedergelassenen \u00c4rzten und Zahn\u00e4rzten ist der Rollout schon weit fortgeschritten: Mehr als acht von zehn Praxen der haus\u00e4rztlichen Versorgung sind nach den j\u00fcngsten Erhebungen der Kassen\u00e4rztlichen Bundesvereinigung (KBV PraxisBarometer Digitalisierung 2020) bereits an die TI angeschlossen. \u00dcberdurchschnittlich gut angebunden sind niedergelassene \u00c4rzte und Psychotherapeuten unter 50 Jahren sowie mittelgro\u00dfe bis gro\u00dfe Praxen.<\/p>\n<\/div><\/section><\/div><\/div><!--close column table wrapper. Beim laufenden Betrieb nach der TI-Anbindung zeigt das System noch eine relativ hohe Fehleranf\u00e4lligkeit: Mehr als jede dritte Praxis (und rund 40 Prozent der gr\u00f6\u00dferen) berichtet im KBV PraxisBarometer von mindestens w\u00f6chentlichen Fehlern im Zusammenhang mit der TI-Nutzung \u2013 die meisten betreffen den Konnektor, das Kartenterminal oder den VPN-Zugang.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die niedergelassenen Apotheker sind ebenfalls schon zum gro\u00dfen Teil an die TI angebunden. Laut einer Auswertung des Deutschen Apothekerverbands (DAV) auf Basis von Daten aus Apothekensoftwareh\u00e4usern und Landesapothekerkammern verf\u00fcgten im M\u00e4rz 2021 drei Viertel der rund 14.000 Apotheken hierzulande \u00fcber einen Anschluss. Die TI-Anbindung der Apotheken ist eine notwendige Voraussetzung f\u00fcr reibungslose Prozesse rund um das eRezept, das derzeit in Berlin-Brandenburg pilotiert und ab Januar 2022 bundesweit f\u00fcr gesetzlich Versicherte verpflichtend eingef\u00fchrt wird.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die Anbindung der Krankenh\u00e4user vollzieht sich gerade erst. Sie sind seit Januar dieses Jahres zum TI-Anschluss verpflichtet und m\u00fcssen ab 2022 mit Sanktionen rechnen, wenn es bis dahin noch nicht geschehen ist. Dies k\u00f6nnte helfen, die bisher immer noch eingeschr\u00e4nkte digitale Kommunikation zwischen Arztpraxen, Krankenh\u00e4usern und anderen Gesundheitseinrichtungen zu erh\u00f6hen. Die letztj\u00e4hrige Analyse im eHealth Monitor hatte ergeben, dass erst 44% der Gesundheitseinrichtungen eine einrichtungs\u00fcbergreifende Patientenakte verwenden und 93% der niedergelassenen \u00c4rzte noch in Papierform mit Krankenh\u00e4usern kommunizieren. Diese Zahlen haben sich nicht wesentlich verbessert, im Gegenteil: Bei den \u00c4rzten zeigt sich im aktuellen KBV PraxisBarometer sogar eine leichte Verschlechterung \u2013 95% kommunizieren mehrheitlich oder nahezu komplett in Papierform (s. Abb. 3). Gegen\u00fcber der letzten Ver\u00f6ffentlichung im eHeath Monitor hat sich dieser Indikator jedoch kaum bewegt. Nach den j\u00fcngsten Erhebungen von KBV (PraxisBarometer Digitalisierung 2020) und HIMSS (eHealth Trend Barometer 2021) stieg die Digitalisierungsrate von Patientendaten in ambulanten und station\u00e4ren Gesundheitseinrichtungen gegen\u00fcber dem Vorjahr geringf\u00fcgig um 1 Prozentpunkt auf jetzt 70%. Im ambulanten Sektor allein ist sie sogar um 2 Prozentpunkte auf 42% gesunken, was allerdings auch durch die Erhebungsmethodik begr\u00fcndet sein kann.<br \/>\n<br class=\"avia-permanent-lb\" \/>Ihre Patientendaten nahezu vollst\u00e4ndig digitalisiert haben bislang 42% der von der KBV befragten Praxen, wobei die Unterschiede zwischen den Fachgruppen gro\u00df sind: W\u00e4hrend mehr als die H\u00e4lfte der \u00e4rztlichen Praxen (56%) ihre Patientendokumentation inzwischen komplett elektronisch abwickeln, tun dies unter den psychotherapeutischen Praxen erst 6%. Innerhalb der niedergelassenen \u00c4rzteschaft steigt die Digitalisierungsrate mit der Praxisgr\u00f6\u00dfe. Mehr als zwei Drittel (67%) der Gemeinschaftspraxen mit mehr als f\u00fcnf \u00c4rzten verf\u00fcgen \u00fcber eine vollst\u00e4ndig digitalisierte Patientendokumentation \u2013 nur bei 7% \u00fcberwiegt noch die Papierform. In allen \u00fcbrigen befragten Regionen (Italien, Niederlande, \u00d6sterreich, Schweiz, Skandinavien, Spanien) geben sich die Einrichtungen Werte \u00fcber 6 Punkte. Skandinavien f\u00fchrt das Ranking mit 6,8 Punkten an, dicht gefolgt von den Niederlanden. Beide gelten auch in Bezug auf die rechtlichen Rahmenbedingungen und den Standardisierungsfortschritt als f\u00fchrend im internationalen Vergleich.
