weitgehend vorangeschritten \u2013 das Gesundheitswesen hinkt nach wie vor hinterher. Bundesgesundheitsminister Jens Spahn hat zu Beginn der aktuellen Legislaturperiode gleichsam einen Sprint vollzogen, um diesen R\u00fcckstand zumindest zu verringern. Mit dem Digitale-Versorgung-Gesetz (DVG), das am 19. Dezember 2019 in Kraft trat, wurden die Weichen f\u00fcr innovative Gesundheitsleistungen gestellt, die Deutschland weltweit in eine Vorreiterrolle bringen: Digitale Gesundheitsanwendungen (DiGA) k\u00f6nnen offiziell von \u00c4rztinnen und \u00c4rzten verschrieben werden und treten somit als neuer Therapiebaustein an die Seite von Arzneimitteln und Medizinprodukten. Mit dem sogenannten DiGA-Fast-Track-Verfahren sorgte die Politik zudem daf\u00fcr, dass das hohe Tempo, das sie selbst vorgelegt hat, nicht auf dem steinigen Pfad durch die Gremien der Selbstverwaltung gedrosselt wird. Gut zehn Monate nach Inkrafttreten des DVG gelangten die ersten beiden DiGA in die Erstattungsf\u00e4higkeit \u2013 f\u00fcr das deutsche Gesundheitswesen ist das Rekordzeit. Mittlerweile wurden 20 Anwendungen in das DiGA-Verzeichnis des Bundesinstituts f\u00fcr Arzneimittel und Medizinprodukte (BfArM) aufgenommen (Stand Ende August 2021).<br \/>\n<br class=\"avia-permanent-lb\" \/>Mehr Patientenorientierung, mehr Teilhabe am Versorgungsprozess sowie eine st\u00e4rker personalisierte und individualisierte Medizin z\u00e4hlen zu den wichtigsten Vorteilen, die man sich von der digitalen Transformation des Gesundheitswesens verspricht. Zudem kann die Sammlung und Auswertung von Daten langfristig enorm zur Versorgungsverbesserung beitragen. Nicht zuletzt spielen die Vernetzung und die Entlastung von Leistungserbringenden eine wichtige Rolle: Denn angesichts des demografischen Wandels, der Zunahme von Multimorbidit\u00e4t und des Fachkr\u00e4ftemangels f\u00fchrt auch im Gesundheitswesen kein Weg daran vorbei, Effizienzgewinne mithilfe von digitalen L\u00f6sungen zu realisieren. Dennoch ist zu konstatieren, dass die Verordnungszahlen der bislang zugelassenen DiGA (aktuell etwa 20.000 laut Health Innovation Hub 2021) vielfach hinter den Erwartungen zur\u00fcckbleiben, die Politik und Hersteller daran gekn\u00fcpft haben. Viele Start-ups mussten ern\u00fcchtert feststellen, dass zwischen \u201everschreibbar\u201c und \u201everschrieben werden\u201c eine riesige L\u00fccke klafft. Angesichts der angespannten Finanzlage, die sich insbesondere vor dem Hintergrund der kostentreibenden Gesetzgebung der letzten Legislaturperiode sowie der Corona-Pandemie bei den Krankenkassen anbahnt, kann dies nur als Signal zum Handeln verstanden werden. Ansonsten l\u00e4uft die DiGA Gefahr, im Zuge der ersten Einsparungswelle zum gut gemeinten Rohrkrepierer zu werden.<\/p>\n<p><br class=\"avia-permanent-lb\" \/>Die ersten Erfahrungen zeigen: Der Leistungsanspruch allein bringt die DiGA noch nicht zu den Patientinnen und Patienten. Der Gesetzgeber setzt einerseits auf den klassischen Distributionsweg \u00fcber die \u00e4rztliche Verordnung, das Prozedere entspricht damit dem f\u00fcr Medikamente, Medizinprodukte oder Heil- und Hilfsmittel. Andererseits hat er einen neuen Zugangsweg \u00fcber die Krankenkassen geschaffen, sodass Patientinnen und Patienten sogar ganz ohne \u00e4rztliche Intervention eine DiGA nutzen k\u00f6nnen. F\u00fcr ein so vollkommen neues Versorgungselement wie die DiGA reicht es aber nicht aus, eine Reihe von Produkten auf den Markt zu bringen und auf selbstregulierende Kr\u00e4fte zu vertrauen \u2013 zumal die Beharrungskr\u00e4fte im Gesundheitswesen bekanntlich stark sind. Insbesondere Hausarztpraxen sind keine Institutionen, die auf schnelle Ver\u00e4nderungen ausgerichtet sind. Hier setzt man eher auf jahrelang eingespielte Verfahren im Behandlungsalltag. Eine DiGA kommt dort einem grundlegenden Kulturwandel gleich. Letztere sehen Fragen der Finanzierung (59%) und die regulatorischen Rahmenbedingungen (48%) eher als Hindernisse an.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die Akzeptanz der Leistungserbringenden betrachten lediglich 36% derjenigen, die selbst in der Versorgung t\u00e4tig sind, als Herausforderung, w\u00e4hrend alle anderen Befragten diesen Punkt deutlich kritischer sehen. F\u00fcr die in der Versorgung T\u00e4tigen fallen dagegen Fragen der Finanzierung mit 38% st\u00e4rker ins Gewicht. Letzteres kann auch als Hinweis auf eine Problematik gelesen werden, den die DiGA aus der Perspektive der Leistungserbringenden hat: Eine Verg\u00fctung \u00fcber die reine Verschreibung hinaus ist f\u00fcr die \u00c4rztinnen und \u00c4rzte bei der DiGA derzeit nicht vorgesehen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Dass die Patientinnen und Patienten eine Digitalisierungsbremse darstellen, glaubt \u00fcbrigens kaum jemand: Deren Akzeptanz (6%), Technikaffinit\u00e4t (4%) und technische Ausstattung (3%) rangieren in der Liste der gr\u00f6\u00dften Herausforderungen von Digital Health ganz unten.<\/p>\n<\/div><\/section><\/div><\/div><!--close column table wrapper. Die Politik erreicht mit 57% immerhin Rang zwei, w\u00e4hrend Krankenkassen mit 40% auf Platz drei stehen. Die geringste Schubkraft geht nach Einsch\u00e4tzung der Befragten von den \u00c4rztinnen und \u00c4rzten aus (4%). Auch dieses Ergebnis liefert Hinweise auf die Gemengelage, in der die DiGA sich aktuell wiederfindet: Die Hersteller dr\u00e4ngen mit Elan und Innovationskraft in den Markt \u2013 \u00fcber ein mangelndes Angebot an DiGA braucht man sich also kaum zu sorgen. Mitgenommen werden m\u00fcssen dagegen die \u00c4rztinnen und \u00c4rzte, die gleichsam als Flaschenhals zwischen den Herstellern und den \u201eEndkunden\u201c \u2013 sprich: den Patientinnen und Patienten \u2013 fungieren.<br \/>\n<br class=\"avia-permanent-lb\" \/>Den gr\u00f6\u00dften Nutzen von Digital-Health-L\u00f6sungen verorten die Befragten klar bei den Patientinnen und Patienten (83%), mit einigem Abstand gefolgt von den \u00c4rztinnen und \u00c4rzten (51%). Die Leistungserbringenden selbst sehen als Profiteure allerdings eher Krankenkassen sowie IT-Unternehmen und weitere Hersteller von Digital-Health-L\u00f6sungen (im Folgenden Digitalunternehmen genannt); erst auf den Pl\u00e4tzen vier und f\u00fcnf folgen in ihren Augen \u00c4rztinnen und \u00c4rzte sowie Krankenh\u00e4user (s. Der Wunsch nach der Aufnahme in Behandlungsleitlinien, um die jeweilige DiGA fest im Versorgungsprozess der jeweiligen Indikation zu verankern, spiegelt ebenfalls die hohe Bedeutung von Evidenz wider.<br \/>\n<br class=\"avia-permanent-lb\" \/>Verbl\u00fcffend ist eher, dass die Zahlen f\u00fcr beide Ma\u00dfnahmen nicht noch deutlich h\u00f6her ausgefallen sind. Aufschlussreich ist hier ein Blick in die Ergebnisse der unterschiedlichen Teilnehmergruppen. W\u00e4hrend die Evidenzgenerierung bei den Krankenkassen auf 70% der Nennungen kommt, liegt sie bei den IT- und Digitalunternehmen lediglich bei 41%. Das zeigt, dass die neuen Player im Gesundheitssystem mit dessen Regelwerk noch nicht in vollem Umfang vertraut sind: Evidenz ist die W\u00e4hrung der Erstattungsf\u00e4higkeit. Der Erfolg auf dem GKV-Markt abh\u00e4ngig vom Zusatznutzen. Und da die Akzeptanz unter den Leistungserbringenden ohnehin als H\u00fcrde f\u00fcr Digital Health gesehen wird, w\u00e4re die Hebelwirkung gro\u00df, wenn man sie \u00fcber die DiGA in den digitalen Transformationsprozess st\u00e4rker einbeziehen k\u00f6nnte.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die Leistungserbringenden selbst halten Schulungen allerdings nur zu 38% f\u00fcr wichtig. Das mag dem Umstand geschuldet sein, dass DiGA-Schulungen f\u00fcr sie nur eine weitere Verpflichtung in ihrem ohnehin schon \u00fcbervollen Terminkalender w\u00e4ren. Vielleicht h\u00e4ngt der vergleichsweise niedrige Wert aber auch damit zusammen, dass \u00c4rztinnen und \u00c4rzte sich selbst insgesamt nicht als Profiteure von Digital Health betrachten und ihre Bereitschaft, eine DiGA zu verschreiben, mit 63% auch nicht allzu ausgepr\u00e4gt ist (Radic et al. 2021). Verwunderlich ist eher, dass deren Einbindung in die elektronische Akte nicht mehr Zustimmung erh\u00e4lt. Bei den Teilnehmenden der Pharma- und Medizintechnikindustrie votierten 49% daf\u00fcr, bei den Leistungserbringenden sind es sogar nur 33%. Dieser niedrige Wert k\u00f6nnte allerdings auch darin begr\u00fcndet sein, dass die ePA bei vielen \u00c4rztinnen und \u00c4rzten generell kein hohes Ansehen genie\u00dft: Einerseits fehlt bisher das Vertrauen in ihren Mehrwert, andererseits f\u00fcrchten viele, dass damit zus\u00e4tzliche Dokumentationspflichten einhergehen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Dabei bietet die Integration von DiGA in die ePA zahlreiche Vorteile: Sie w\u00e4ren besser in ein therapeutisches Gesamtkonzept eingebunden und Behandlungseffekte deutlicher erkennbar. Zudem k\u00f6nnte auf diese Weise ein wertvoller Pool an Versorgungsdaten entstehen, der sowohl in die Gestaltung innovativer Versorgungsmodelle als auch in einen Erstattungsbetrag f\u00fcr DiGA einflie\u00dfen k\u00f6nnte. Dass eine gro\u00dfe Mehrheit der Befragten vom Mehrwert der DiGA f\u00fcr die Patientinnen und Patienten \u00fcberzeugt ist, ist eine gute Ausgangsbasis. Es reicht aber nicht, um das DiGA-Konzept zum Fliegen zu bringen. Ein notorischer Webfehler unseres Gesundheitssystems ist, dass keine Nutzenszenarien aus der Perspektive der beteiligten Akteure entworfen werden. Mit anderen Worten: Es fehlt der DiGA an einem durchdachten Gesch\u00e4ftsmodell. Das betrifft aber nicht sie allein \u2013 Gesch\u00e4ftsmodelle f\u00fcr Innovationen sind im F\u00fcnften Sozialgesetzbuch schlicht nicht vorgesehen.<br \/>\n<br class=\"avia-permanent-lb\" \/>F\u00fchrt man sich den Vertriebsapparat vor Augen, der bei Pharmaunternehmen in Gang gesetzt wird, um ein neues Medikament im Markt zu platzieren, wirken DiGA geradezu vernachl\u00e4ssigt. Es ist klar, dass die meisten Start-ups hier finanziell nicht mithalten k\u00f6nnen. Umso kreativer gilt es deshalb zu werden, um neben Patientinnen und Patienten vor allem Kostentr\u00e4ger und Leistungserbringende von ihrem Produkt zu \u00fcberzeugen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Die Kommunikation von Evidenznachweisen muss dabei h\u00f6chste Priorit\u00e4t haben (vgl. L\u00e4gel 2020c). Die Aufnahme in Behandlungsleitlinien w\u00e4re dar\u00fcber hinaus der Goldstandard, der auch auf Krankenkassen eine \u00fcberaus beruhigende Wirkung h\u00e4tte. F\u00fcr die Kostentr\u00e4ger ist die DiGA in dem Moment interessant, in dem sie beweist, dass sie ihr Geld wert ist, weil sie zu Einsparungen an anderer Stelle f\u00fchrt. Das w\u00e4re beispielsweise der Fall, wenn sie dazu beitr\u00e4gt, Chronifizierungen zu vermeiden, Krankheitsepisoden zu verk\u00fcrzen oder die Steuerung von Patientinnen und Patienten durch das System zu verbessern. Ein gro\u00dfes Potenzial k\u00f6nnten DiGA aus der Perspektive der Krankenkassen entfalten, wenn sich durch sie Versorgungsengp\u00e4sse abfedern lie\u00dfen, (beispielsweise im Bereich psychischer Erkrankungen) oder \u00dcberg\u00e4nge von station\u00e4ren in ambulante Versorgungsstrukturen nahtloser gestaltet werden k\u00f6nnten (etwa im Bereich Physiotherapie nach einer Reha, bei Suchterkrankungen oder bei der Etablierung von Lebensstil\u00e4nderungen).<br \/>\n<br class=\"avia-permanent-lb\" \/>Leistungserbringende wiederum sind, wie das Innovationspanel klar gezeigt hat, bislang am schwersten von digitalen Gesundheitsl\u00f6sungen zu \u00fcberzeugen. Also gilt: Bei aller Liebe zu den Patientinnen und Patienten sollten DiGA-Hersteller den Nutzen f\u00fcr die \u00c4rztinnen und \u00c4rzte nicht ganz aus dem Blick verlieren. Eine DiGA mag noch so beeindruckende Funktionen haben: Erzeugt sie bei den Versorgenden sp\u00fcrbaren Mehraufwand, streben ihre Erfolgschancen gegen Null (Wangler u. Jansky 2020). Erschwerend kommt hinzu, dass f\u00fcr die Verordnung einer DiGA genau wie bei Arzneimitteln und anderen Medizinprodukten keine gesonderte Verg\u00fctung vorgesehen ist. Im Umkehrschluss hei\u00dft das: Das gr\u00f6\u00dfte Potenzial haben diejenigen DiGA, die gleichzeitig zu einer verbesserten Versorgung und zu einer Zeitersparnis f\u00fcr die Versorgenden f\u00fchren.<br \/>\n<br class=\"avia-permanent-lb\" \/>Zudem m\u00fcssen die Hersteller aktiv auf \u00c4rztinnen und \u00c4rzte zugehen und ihnen die Informationen \u00fcber das Produkt mundgerecht servieren, sei es im Rahmen von Schulungen oder \u00fcber traditionelle Vertriebswege: Denn digitale Kommunikation mag in den DiGA-Schmieden selbstverst\u00e4ndlich sein \u2013 in vielen Arztpraxen ist sie es noch nicht. DiGA sollten die Chance erhalten, sich dieses Vertrauen gleicherma\u00dfen zu erwerben. DiGA bieten hier eine gro\u00dfe Chance, denn derzeit l\u00e4sst sich kaum eine Therapieform flexibler in den Alltag einbetten als die, die man in Form des Smartphones in der Hosentasche mitf\u00fchrt.<br \/>\n<br class=\"avia-permanent-lb\" \/>Eine wichtige Strategie d\u00fcrfte f\u00fcr die Hersteller deshalb zum einen in der Beteiligung an integrierten Versorgungsmodellen liegen. Zum anderen kann die DiGA Mehrwert \u201ebeyond\u201c und \u201earound the pill\u201c generieren \u2013 also um die klassische Arzneimittelversorgung herum. Denn das beste Medikament ist wirkungslos, wenn es nicht zum richtigen Zeitpunkt in der richtigen Dosis eingenommen beziehungsweise angewendet wird (Amelung u. Ex 2019b).<br \/>\n<br class=\"avia-permanent-lb\" \/>Mit anderen Worten: Das Erfolgsrezept f\u00fcr eine zukunftsorientierte Gesundheitsversorgung, in der auch die DiGA ihren Beitrag leistet, d\u00fcrfte in der Gestaltung von Wertsch\u00f6pfungsketten liegen. Dieser Ansatz wird aber nur funktionieren, wenn er von neuen Finanzierungsmodellen flankiert wird, beispielsweise in Form von Bundled Payments. Eine Verg\u00fctung von definierten Episoden der Versorgung, etwa f\u00fcr die Diabetesversorgung innerhalb eines F\u00fcnfjahreszeitraums, bietet den Vorteil, dass die Versorgung aus individuell angepassten Bausteinen konzipiert werden kann. Das schafft Planungssicherheit bei den Leistungserbringenden und l\u00e4sst gleichzeitig Raum f\u00fcr Innovationen, weil die Versorgenden unmittelbar von Verbesserungen partizipieren, sei es in Form von Entlastung durch bessere Prozesse oder in monet\u00e4rer Hinsicht, weil Kosten eingespart werden.<br \/>\n<br class=\"avia-permanent-lb\" \/>Digitale Produkte stellen in solchen Szenarien unverzichtbare Bausteine dar. Sie k\u00f6nnen Wertsch\u00f6pfungsketten erweitern und die einzelnen Versorgungskomponenten miteinander vernetzen.
