Gesundheitsdienste<\/span> werden von Krankenh\u00e4usern, Arztpraxen, Apotheken und gesetzlichen Krankenversicherungen (GKVen) zunehmend angeboten. Gleichzeitig zielen regulatorische Initiativen wie das Krankenhauszukunftsgesetz auf eine weitere Digitalisierung der Einrichtungen. Der Umsetzungsfortschritt indessen l\u00e4sst an einigen Stellen noch auf sich warten \u2013 sowohl im station\u00e4ren als auch im ambulanten Bereich. Einrichtungs\u00fcbergreifend aber setzt sich die Etablierung digitaler Dienstleistungen im medizinischen Versorgungsalltag weiter fort, wie die Analysen in diesem Kapitel des deutschen Gesundheitssystems wird ma\u00dfgeblich von Leistungserbringern wie Krankenh\u00e4usern, Arztpraxen, Telemedizinanbietern und Apotheken vorangetrieben. Insgesamt stehen \u00c4rztinnen und \u00c4rzte in Kliniken und Praxen Digitalisierung des Gesundheitswesens positiv gegen\u00fcber, wie eine Umfrage des Branchenverbands Bitkom aus dem Jahr 2022 ergab: Drei Viertel (76%) betrachten die Digitalisierung inzwischen als Chance f\u00fcr das Gesundheitswesen \u2013 deutlich mehr als noch zwei Jahre zuvor (2020: 67%). Als Risiko sehen sie hingegen nur noch 22% (2020: 27%)<sup style=\"line-height: 0.5em;\">1<\/sup>.<br \/>\n<br class=\"avia-permanent-lb\" \/>Trotzdem schreitet die Implementierung digitaler Angebote eher langsam voran, da nach wie vor insbesondere technische H\u00fcrden zu \u00fcberwinden sind. Zwei von drei Befragten in der \u00c4rzteschaft fordern inzwischen mehr Tempo bei der Beseitigung dieser Hindernisse \u2013 ein Anstieg um 10 Prozentpunkte seit 2020. Hierf\u00fcr stellen Bund, L\u00e4nder und Tr\u00e4ger f\u00fcr elf F\u00f6rdertatbest\u00e4nde bis zu 4,3 Mrd. EUR \u00fcber den Krankenhauszukunftsfonds (KHZF) zur Verf\u00fcgung. Das Programm unterst\u00fctzt neben dem Ausbau der digitalen Notaufnahme insbesondere Investitionen in die Digitalisierung und IT-Sicherheit. In vielen Krankenh\u00e4usern verl\u00e4uft die Umsetzung allerdings eher schleppend \u2013 auch aufgrund des Mangels an IT-Fachkr\u00e4ften und der Geschwindigkeit der Auszahlung aus dem KHZF.<br \/>\n<br class=\"avia-permanent-lb\" \/>Der GKV-Spitzenverband (GKV-SV) und die Deutsche Krankenhausgesellschaft (DKG) haben sich deshalb darauf geeinigt, dass die gesetzliche Frist f\u00fcr den Abschluss von verpflichtenden Digitalprojekten verl\u00e4ngert wird. Mussten Krankenh\u00e4user urspr\u00fcnglich bis Ende 2024 die Nutzung der neuen Angebote nachweisen, um Sanktionen zu vermeiden, reicht es nun, die Digitalprojekte bis zu diesem Zeitpunkt lediglich in Auftrag zu geben. F\u00fcr die Inbetriebnahme greift dann ein Stufenplan: 60% Mindestnutzungsquote bis Ende 2027, 70% bis 2028 und 80% bis 2031. Gemessen wird der Digitalisierungsfortschritt in den Krankenh\u00e4usern anhand eines Soll-Ist-Abgleichs.<br \/>\n<br class=\"avia-permanent-lb\" \/>Der geringe Umsetzungsfortschritt wirkt sich auf nahezu das gesamte digitale Angebot der Krankenh\u00e4user aus. Einzige Ausnahme bildet das Patienten-WLAN: Mit einer Verf\u00fcgbarkeit in 71% der Krankenh\u00e4user ist es die mit Abstand verbreitetste Technologie. Ansonsten gehen Angebot und Nachfrage deutlich auseinander \u2013 das zeigt eine Bitkom-Umfrage von 2022 unter 166 \u00c4rztinnen und \u00c4rzten, die in deutschen Krankenh\u00e4usern arbeiten (s. Abb. 1). Zwar w\u00fcnschen diese sich mehrheitlich digitale L\u00f6sungen, um einerseits Versicherten den Zugang zur Gesundheitsversorgung zu erleichtern und andererseits die Betriebseffizienz zu verbessern. Doch bislang kommen sie kaum zum Einsatz: So nutzt erst rund ein F\u00fcnftel der Kliniken digitale Verwaltungstechnologien wie die tabletgest\u00fctzte Patientenaufnahme (18%) oder digitale Aufkl\u00e4rungsb\u00f6gen (20%), obgleich rund zwei Drittel der \u00c4rzteschaft dies bef\u00fcrworten.<br \/>\n<br class=\"avia-permanent-lb\" \/>Oben auf der Wunschliste des medizinischen Krankenhauspersonals steht zudem die telemedizinische Konsultation anderer \u00c4rztinnen und \u00c4rzte: Die Mehrzahl (57%) h\u00e4lt die Technologie f\u00fcr sinnvoll, doch erst ein knappes Drittel kann sie auch nutzen. Doch immer mehr Praxisbetriebe glauben an das Potenzial einer besseren Versorgung durch digitalisierte inner\u00e4rztliche Kommunikation. Das best\u00e4tigt eine KBV-Umfrage von 2022 unter 2.459 Befragten in der \u00c4rzteschaft und Psychotherapie: 70% sehen einen Anwendungsnutzen beim Arztbrief (2021: 65%) und 58% bei Befunddaten (2021: 55%).<\/p>\n<\/div><\/section><\/div><\/div><!--close column table wrapper. Ein Grund hierf\u00fcr liegt in der Regulatorik: Konnten die Praxen in der Akutphase von COVID-19 noch unbegrenzt Videosprechstunden anbieten und abrechnen, sind seit April 2022 Fallzahl und Leistungsmenge wieder begrenzt, wenn auch auf 30% statt wie vor der Pandemie auf 20%. Nur 3% nutzen sie aufgrund fehlender Mobilit\u00e4t. Gerade in l\u00e4ndlicheren Gegenden und Kleinst\u00e4dten ist die Akzeptanz hoch. Laut einer Forsa-Befragung im Auftrag der Techniker Krankenkasse bef\u00fcrworten neun von zehn Menschen in Hessen telemedizinisch begleitete Hausbesuche.<br class=\"avia-permanent-lb\" \/>Die digitale Kommunikation mit Patientinnen und Patienten au\u00dferhalb der Praxis steigt laut KBV PraxisBarometer weiterhin an: Der Anteil nahezu komplett oder mehrheitlich digitaler Interaktion erh\u00f6ht sich in Arztpraxen gegen\u00fcber dem Vorjahr um 6% auf jetzt 36%. Und fast alle (78%) setzen das digitale Verfahren inzwischen in der Praxis ein. Der Grund: Seit dem 1. Juli 2023 k\u00f6nnen E-Rezepte bundesweit sowohl mit der elektronischen Gesundheitskarte (eGK) als auch in Papierform mit QR-Code eingel\u00f6st werden. F\u00fcr die \u00c4rzteschaft verpflichtend wird die digitale Rezeptausstellung erst nach Inkrafttreten des DigiG Anfang 2024. Doch schon in der zweiten Jahresh\u00e4lfte 2023 zeigte sich ein deutlicher Nutzungsanstieg: Lag die Zahl der eingel\u00f6sten E-Rezepte 2022 noch unter 890.000 (das entspricht 0,1% der insgesamt 775 Millionen GKV-Verordnungen pro Jahr), waren es Ende September 2023 schon 4,1 Millionen \u2013 ein Plus von 357%. Allein in den letzten drei Monaten der Erhebung verzeichnete das E-Rezept einen Zuwachs von 84% (s. W\u00e4hrend Online-Programme zur Gesundheitsf\u00f6rderung (z.B. Ern\u00e4hrung, Bewegung, Raucherentw\u00f6hnung) schon seit 2020 von s\u00e4mtlichen gro\u00dfen Kassen angeboten werden, nehmen auch andere digitale GKV-Services zu, die das DFSI untersucht: So bieten 88% dieser Kassen inzwischen Online-Bonusprogramme an \u2013 2022 waren es noch 71%. Das Angebot an Videokommunikation ist hingegen nicht weiter gewachsen: W\u00e4hrend 13 von 17 Versicherungen bereits \u00fcber einen medizinischen Info-Videochat verf\u00fcgen, bieten nur sieben eine erweiterte Online \/Videosprechstunde an.<br \/>\n<br class=\"avia-permanent-lb\" \/>Mehr Bewegung gibt es auf der Versichertenseite: Fast zwei von drei GKV-Mitgliedern (63%) m\u00f6chten den Gro\u00dfteil ihrer Krankenkassen-Angelegenheiten zuk\u00fcnftig auf digitalem Weg regeln. 2021 waren es noch 55%. Das ergab eine Umfrage des Techmonitor GKV, die das Marktforschungs- und Beratungsinstitut HEUTE UND MORGEN 2022 unter 1.500 Versicherten durchgef\u00fchrt hat. Trotzdem bleiben pers\u00f6nliche Ansprechpersonen f\u00fcr 61% der Befragten weiterhin wichtig. Grunds\u00e4tzlich steht die H\u00e4lfte der Versicherten der fortschreitenden Digitalisierung in der GKV aufgeschlossen gegen\u00fcber. Ein Viertel w\u00fcnscht sich sogar eine Erweiterung der bestehenden digitalen Angebote. Besonders gefragt sind Apps, die verschiedene zentrale Service- und Kommunikationsbereiche miteinander verkn\u00fcpfen und vereinfachen.<br \/>\n<br class=\"avia-permanent-lb\" \/>Einen Neustart gibt es f\u00fcr die elektronische Patientenakte (ePA) durch die Regierungsentscheidung f\u00fcr ein Opt-out-Verfahren, bei der die Versicherten einer Nutzung aktiv widersprechen m\u00fcssen. Zum Stand Ende September 2023 waren gut 800.000 ePA aktiviert. Das entspricht zwar einem Plus von 7% gegen\u00fcber dem Vorjahr (knapp \u00fcber 544.000), dennoch nutzen damit immer noch nur 1% der gesetzlich Versicherten in Deutschland eine ePA. Zwar wissen fast alle inzwischen, was die ePA ist, und eine Mehrheit (59%) m\u00f6chte sie auch nutzen, wie eine Bitkom-Befragung ergab. Allerdings wollen 73% besser \u00fcber die ePA informiert werden und fast sechs von zehn Befragten sorgen sich um die Sicherheit ihrer Daten.<br \/>\n<br class=\"avia-permanent-lb\" \/>Mithilfe der Opt-out-L\u00f6sung strebt das Bundesgesundheitsministerium (BMG) nun eine Nutzerquote von 80% bis 2025 an. Regelungen zur vereinfachten Datenfreigabe sowie zu einer nutzerfreundlichen Steuerung in der ePA-App sind derzeit noch in Arbeit.
